    Emergency Medical Card

    This web site allows you to enter all your personal information and writes out an emergency medical card for you. Simply fill out the form with all your pertinent information and print it out on your computer printer. In fact, it might be a good idea to print out several of them. Put one in your wallet, one in your glove box and one near your main telephone at home (putting it in the front of your phone book is a good idea for safe keeping).
